What needs to be fill out?

The minimum information needed for personal or individual use is:

• Box 3 - Indicate this is for Personal/Individual use only
• No business entity or individual may receive mail to this address.
• Box 4 - Name of Applicant
• Please fill this out completely and accurately. Any inaccurate details will delay the application process.
• Box 8 - Photo ID Information
• This must be a valid, non-expired photo id.
• We recommend a Driver License, State ID, Uniformed Services ID, or Public University ID
• Box 9 - Address ID Information
• This must be a valid non-expired document that matches the name of the applicant with their personal residential address.
• This address must match the address provided in Box 4 - Name of Applicant. If they do not match, the form is invalid and will need to be corrected.
• We recommend the use of a Passport as the expiration date is typically longest.

• Please fill out this information completely and accurately. Please use the physical address of business / organizations operations. Note: this address is not for mailing purposes. The USPS requires this address for identification and verification purposes.
• Box 8 - Photo ID Information
• This must be a valid, non-expired photo id.
